Heart Health Check

120 minutes

£120

Your heart works continuously for you. Often, early changes in heart health develop quietly, without obvious symptoms.

A heart health check offers the opportunity to assess your cardiovascular wellbeing in a thorough, reassuring and clinically informed way.

This appointment is suitable if you:

  • Have a family history of heart disease

  • Have high blood pressure, raised cholesterol or diabetes

  • Experience palpitations, breathlessness or chest discomfort

  • Feel unusually fatigued

  • Want reassurance and proactive screening

  • Have recently been diagnosed with a heart condition

What the Appointment Includes

Your assessment begins with a detailed discussion of your medical history and current symptoms. We explore:

  • Personal and family cardiac history

  • Blood pressure history

  • Cholesterol and metabolic risk factors

  • Lifestyle factors including sleep, nutrition, stress and physical activity

  • Smoking status and alcohol intake

  • Current medications

Understanding the full context of your health allows for a more accurate and meaningful assessment.

Clinical Examination

Your heart health check includes:

  • Blood pressure measurement

  • Resting heart rate assessment

  • Oxygen saturation levels

  • Cardiac and respiratory examination

  • 12 lead electrocardiogram ECG

The ECG records the electrical activity of your heart. It can help identify heart rhythm abnormalities, previous silent cardiac events, conduction disturbances and other changes that may require further investigation.

The procedure is painless and takes only a few minutes.

After Your Assessment

Once your examination and ECG have been reviewed, we will discuss the findings in clear and understandable terms.

You will receive:

  • A written summary of your results

  • An explanation of any identified risk factors

  • Personalised advice on lifestyle modification where appropriate

If any abnormalities are detected or further investigation is required, we will talk about this with you as soon as possible. With your consent, you can go ahead and arrange a referral to your registered GP or an appropriate specialist. Your written report can be shared directly with your GP to support continuity of care.

Why Book a Heart Health Check

Cardiovascular disease remains one of the leading causes of illness, yet many risk factors are modifiable. Early detection and prevention are key.

A heart health check provides reassurance where all is well, and clarity and direction where change is needed. It is a proactive step towards protecting your long-term health.

If you are unsure whether this assessment is suitable for you, you are welcome to make contact to discuss your individual circumstances.

Health & wellbeing assessment

120 minutes

£150

There are times when you do not feel acutely unwell, yet you know you are not thriving.

You may feel tired, run down or uncomfortable in your body. Your fitness may have declined.

You may be living with ongoing aches, poor sleep or low energy. Nothing feels urgent, yet something does not feel right. When appointments feel rushed or difficult to access, it is easy to normalise these symptoms and attribute them to stress or ageing. This comprehensive assessment offers protected time to explore your health in depth with an experienced nurse and holistic practitioner. It is an opportunity to step back, review the bigger picture and gain clarity about what your body may be signalling.

What the Assessment Includes

We begin with a detailed life and health history to understand you as a whole person, not just a set of symptoms.

Your consultation includes exploration of:

  • Existing medical conditions and current symptoms

  • Medications and supplements

  • Family history and patterns of illness

  • Risk factors for chronic disease

  • Sleep quality and patterns

  • Nutrition and hydration

  • Physical activity and movement

  • Stress levels and emotional wellbeing

  • Environmental influences

This whole person approach helps identify connections that are often missed in shorter consultations.

Physical Examination

Your appointment also includes a focused clinical examination:

  • Blood pressure measurement

  • Heart rate and rhythm assessment

  • Oxygen saturation levels

  • Listening to heart and lung sounds

These observations provide important insight into your cardiovascular and respiratory health and support safe, evidence based recommendations.

Your Personalised Plan

At the end of your assessment, we will agree on a clear and realistic management plan tailored to your needs. This may include lifestyle recommendations, monitoring strategies, further investigations or onward referrals if appropriate.

Any referral or signposting is always discussed with you and undertaken collaboratively.

You will receive a written summary of your results and assessment findings. You may choose to share this with your registered GP or other healthcare professionals to support ongoing care.

Post hospital Nursing discharge assessment

90-120 minutes

£120

Home visit or clinic appointment, depending on clinical need

Being discharged from the hospital can feel unsettling. You may return home with new medications, unanswered questions or uncertainty about what is normal during recovery. Follow-up appointments are not always immediate, and it can be difficult to know who to contact if concerns arise.

This service provides structured, compassionate clinical support during the transition from hospital to home. The aim is to reduce anxiety, monitor your recovery safely, and ensure you feel informed and confident about the next steps.

Who This Service Is For

This appointment is suitable if you:

  • Have recently been discharged from hospital

  • Have new or changed medications

  • Feel unsure about your recovery plan

  • Require monitoring following a cardiac, respiratory or medical admission

  • Would benefit from professional reassurance at home

What the Appointment Includes

Your consultation begins with a review of your recent hospital stay and current care plan. We will discuss:

  • The reason for your admission

  • Treatments received

  • Medication changes

  • Current symptoms or concerns

  • Understanding of your discharge instructions

A focused clinical assessment will then be carried out, which may include:

  • Blood pressure measurement

  • Heart rate and rhythm assessment

  • Oxygen saturation levels

  • Heart and lung auscultation

  • General observation of your recovery progress

This ensures that any early concerns are identified promptly and appropriately managed.

Ongoing Support and Communication

At the end of your appointment, you will receive a written summary outlining:

  • Clinical findings

  • Current observations

  • Recommendations

  • Any advice given

If required, this document can be shared with your registered GP or specialist team to support continuity of care.

Where appropriate, referrals or signposting to other healthcare services will be discussed and arranged in partnership with you.

The Aim of This Service

Recovery does not end at discharge. Early support at home can reduce complications, improve confidence and ensure that small concerns do not become larger problems.

This appointment provides clinical oversight combined with reassurance and clear communication, helping you move forward safely during a vulnerable period.

If you are unsure whether this service is suitable for you or a family member, you are welcome to make contact to discuss your circumstances.
