You may also have been diagnosed with a health condition and want clearer guidance, reassurance and support to manage it confidently. Or perhaps you want to be proactive and understand your health before problems develop.
This assessment gives you protected time and space to explore your health fully with an experienced nurse and holistic health and wellbeing practitioner.
You will also receive a journal and a copy of my manual “How to be a Well Being”.
What the Assessment Includes
We begin with a comprehensive life and health history. This helps us build a clear picture of your current state of health and any potential risks. We explore:
Existing medical conditions and current symptoms
Family history and patterns of illness
Lifestyle factors including sleep, nutrition, movement and stress
Environmental influences
Medications and supplements
Risk factors for chronic disease
This is not a rushed appointment. It is a thorough and thoughtful conversation that looks at the whole of you, not just isolated symptoms.
Physical Assessment (Face to Face Appointments)
If you attend in person, your assessment will also include:
Blood pressure
Heart rate and rhythm assessment
Oxygen saturation levels
Listening to heart and lung sounds
These checks provide valuable clinical insight and allow us to identify areas that may require further investigation.
Your Personalised Plan
Following your assessment, we will agree on a clear and realistic management plan tailored to you. This may include lifestyle adjustments, further investigations, referrals or signposting to other services where appropriate. Any onward referral is always discussed and agreed with you.
You will receive a written summary of your assessment findings and recommendations. You may choose to share this with your registered GP or other healthcare professionals if helpful.

During your initial appointment, we explore:

If you attend in person, your vital signs will also be recorded and discussed. We establish a baseline wellness score and agree on key priority areas for change.
From this, we create a personalised and achievable plan.
FOLLOW-UP Sessions
Your five 60-minute follow-up sessions are spaced according to your needs. These appointments provide time to review progress, adjust your plan and work through barriers in a supportive and structured way.
Throughout the programme, we use coaching tools grounded in positive psychology and behaviour change science. You will explore your beliefs, habits and patterns, and develop practical strategies to strengthen self-awareness, confidence and resilience.
Between sessions, you may receive educational resources such as short videos, guided practices, recommended reading or podcasts to deepen your understanding and support implementation.
We regularly reassess your wellbeing score to track change and ensure you are moving in the right direction.
Clinical Oversight and Referrals
As a nurse led programme, clinical safety remains central. If at any point we identify the need for further investigation or referral, this will be discussed with you and appropriate signposting arranged. You will receive written summaries of your assessment and progress, which you may share with your registered GP if helpful.

You are free to withdraw from the programme at any stage if you feel it is not right for you. I simply ask for 48 hours’ notice if you need to cancel an appointment.

What the Programme Includes
This is a six-session journey is tailored to your needs. Appointments can be face-to-face, on-line or a combination of both.
