Understanding Sleep, Cortisol and Your Circadian Rhythm

Why am I wide awake at 3 am?

If you've found yourself staring at the ceiling somewhere between 2 am and 4 am, heart racing, mind suddenly wide awake for no obvious reason, contemplating what you said 10 years ago, or what might happen tomorrow, and feeling anxious about being over-tired, you are not alone. This is one of the most common complaints I hear in clinic, particularly from women navigating perimenopause and menopause. You are not going mad; there is physiology behind it.

Your body clock and the cortisol curve

Cortisol isn't just a “stress hormone”; it's the hormone that helps wake you up. Under normal circumstances, cortisol follows a predictable daily rhythm, known as the diurnal cortisol curve. It's at its lowest around midnight, and begins rising in the early hours, and peaks shortly after waking to help mobilise energy for the day ahead. This is called the cortisol awakening response.

‍ The trouble is, several things can push that natural rise earlier than it should be, tipping it into the middle of the night instead of the morning. Falling oestrogen and progesterone during perimenopause can disrupt the delicate feedback loop between your hypothalamic-pituitary-adrenal (HPA) axis and your circadian pacemaker (the fancy term for your body clock). Blood glucose dips, alcohol metabolism, erratic bedtimes with late nights, and chronic low-grade stress can all do the same thing. The result is that unmistakable 3 am wake-up, often with a racing heart or a wired, anxious feeling, even when nothing is actually wrong.

‍ High cortisol levels directly affect your melatonin production. Melatonin is your sleep hormone that naturally rises as the light fades. Bright lights trick the brain into thinking it's still daytime. This hinders your sleep quality and quantity. You may find you fall asleep easily as you are exhausted but then wake during the night and have trouble getting back to sleep. This is a sign of a disrupted body clock.

Why this matters for more than just tiredness

Chronically disrupted sleep and elevated nocturnal cortisol don't just leave you exhausted. Over time, they're linked to insulin resistance, higher blood pressure, and increased cardiovascular risk, alongside the more familiar effects on mood, memory and resilience. We have some very cool brain laundry that happens only when we are asleep; this is when the body heals, and it does not happen at any other time during the day. This is why it is so important to protect your sleep.

Gentle, practical sleep hygiene tips

‍None of these is about perfection; they're small shifts that help your body remember what night-time is for.

•  Regular wake times. A consistent wake time, even at weekends, does more for your circadian rhythm than an early bedtime ever will.

•  Get daylight early. Ten to fifteen minutes of natural light within an hour of waking helps set your cortisol and melatonin rhythms for the following 24 hours.

•  Eat earlier. Your body does not need to be digesting food when you are trying to sleep. Avoid heavy meals 2 hours before bed. If you need a snack, rather than a carb-heavy snack, go for something protein-based.

•  Watch alcohol, even small amounts. Alcohol fragments sleep architecture and can contribute directly to those early morning wake-ups.

•  Cool, dark, quiet. A bedroom around 16 to 18°C supports the natural drop in core temperature that sleep onset depends on (challenging during this UK summer!)

•  Wind down on purpose. Twenty minutes of low light, no screens, and something calming (reading, gentle stretching, breathing exercises) signals safety to your nervous system.

•  Cognitive reshuffling. The old method of counting sheep has some evidence behind it. A mental process of changing thoughts can help you get to sleep.

•  If you wake at 3 am, don't fight it. Lying there willing yourself back to sleep raises cortisol further. Try slow, extended exhale breathing, or get up briefly in low light if you're truly wide awake.

Be kind to yourself

Waking at 3 am isn't a sign of weakness or a habit you've failed to break; it's often your physiology responding to real hormonal and lifestyle shifts. Understanding what's happening beneath the surface is usually the first step towards gently working with your body rather than against it.
