Post hospital discharge nursing assessment

Being discharged from the hospital can feel unsettling. You may return home with new medications, unanswered questions or uncertainty about what is normal during recovery. Follow-up appointments are not always immediate, and it can be difficult to know who to contact if concerns arise.

This service provides structured, compassionate clinical support during the transition from hospital to home. The aim is to reduce anxiety, monitor your recovery safely, and ensure you feel informed and confident about the next steps.

Who This Service Is For

This appointment is suitable if you:

  • Have recently been discharged from hospital

  • Have new or changed medications

  • Feel unsure about your recovery plan

  • Require monitoring following a cardiac, respiratory or medical admission

  • Would benefit from professional reassurance at home

What the Appointment Includes

Your consultation begins with a review of your recent hospital stay and current care plan. We will discuss:

  • The reason for your admission

  • Treatments received

  • Medication changes

  • Current symptoms or concerns

  • Understanding of your discharge instructions

A focused clinical assessment will then be carried out, which may include:

  • Blood pressure measurement

  • Heart rate and rhythm assessment

  • Oxygen saturation levels

  • Heart and lung auscultation

  • General observation of your recovery progress

This ensures that any early concerns are identified promptly and appropriately managed.

Ongoing Support and Communication

At the end of your appointment, you will receive a written summary outlining:

  • Clinical findings

  • Current observations

  • Recommendations

  • Any advice given

If required, this document can be shared with your registered GP or specialist team to support continuity of care.

Where appropriate, referrals or signposting to other healthcare services will be discussed and arranged in partnership with you.
